Office Hours
Our office hours are:
- Monday – Thursday: 8 a.m. to 5 p.m.
- Friday: 8 a.m. to 12 p.m.
We are closed for lunch daily from 12 p.m. to 1 p.m. Emergency coverage is available 24 hours a day, 7 days a week.
Initial Visit
Please bring the following to your initial visit:
- Completed paperwork
- Identification
- Current insurance card(s)
- Written referral, if required
- List of current medications
- Imaging (please obtain a CD copy)
- Copay
